Patience Sibongile Chivaraidze Gosha

Working as a case manager with the clients post injury and major trauma. I support individuals when their life has changed beyond recognition. I work under the instruction of legal sector to support clients during litigation and in some instances post settlement. I assess client’s needs, understanding them pre-injury and identifying their needs post-injury by undertaking intensive lmmediate Needs Assessments. I put plans in place based on the client’s needs for a successful rehabilitation programme with clear, manageable goals, implementing the plans alongside the client (including the parent or carer of children and young people). I coordinate the relevant care and support clients access statutory services, counselling, physiotherapy, pain management clinic’s, acupuncture therapy, financial support as well as educational needs, vocational support, hobbies and lifestyle choices and monitor progress and respond to their changing needs. I liaise with the Consultants and GP on behalf of the clients.

Working as an agency Staff Nurse and currently working on a research project as a Service Delivery Lead with Our Future Heath. I also have the opportunity of seeing clients in their own homes and undertake their blood draws and blood pressure monitoring.

Worked as a Case Manager with clients who suffered major trauma and injury, under the instruction of the legal sector to support clients during litigation and in some instances post settlement. I assessed client's needs by developing a rehabilitation report which included the clients requirements such as physiotherapy, psychological support and financial support. I liaised with the General Practitioner, Consultants and wrote letter on behalf of the client.
